Risk register

A list of identified risks with their owners, assessments and treatments.

What it means

A risk register records what an organisation believes could go wrong, how serious each item is, who owns it and what is being done. It is the most common artefact in risk management and the most commonly abandoned: registers are built for an audit or a board paper, then not revisited, and within two cycles they describe an organisation that no longer exists.

The register is not the discipline. What makes it useful is whether anything moves as a result — whether owners are real, actions close with evidence, and someone notices when they do not.
